The purpose of this on-line registration appointment booking form is to facilitate the immunization of children attending licensed child-care centers or schools who are required to be immunized pursuant to the Immunization of School Pupils Act or Child Care and Early Years Act. Information collected on the form will be used to assess the eligibility of the child/student to receive the vaccines requested, to book an appointment and for statistical gathering respecting the use of this website. Information that is deemed necessary from you will be marked as “required” and must be provided in order to book an immunization appointment through this on-line registration form.

As a result, a small risk may exist to your personal health information being accessed by others without your consent.
